The Patient Navigator provides coordination for all patients at all locations ensuring that they are receiving the correct level of care and works closely with Therapists as a critical member of the FastTrack Team/TreatFirst Team. This individual is also responsible for collecting initial screening and registration data to convert new service requests into scheduled intake assessments and will be assigned with and responsible for the outreach to patients who have not engaged in Behavioral Health services in up to 90 days.


  • Must be at least 18 years of age
  • Bilingual – Spanish speaking – Must pass ALTA verbal test with a score of 9
  • High school diploma or equivalent and a minimum of three years of experience working with the target population;
  • Associate’s degree and a minimum of two years of experience working with the target population; or
  • Bachelor’s degree in a human services field from an accredited university and have one year of relevant experience with the target population
  • Complete a minimum of 20 hours of documented training in Relias within the first 90 days of hire date
  • Must be able to demonstrate sufficient knowledge and understanding of human behavior, substance abuse and behavior management theories.
  • Must be able to provide culturally sensitive services as shown by knowledge, skills and abilities ascertained from initial interview pre-employment
  • Must have excellent listening and observation skills and be capable of establishing rapport with children, families and outside entities.
  • Multi-tasking skills with special attention to organization and detail.
  • Must have strong verbal and written skills
  • Knowledge of Windows environment and/or Electronic Health Records systems.
  • Must have a current CPR & First Aid certification or receive training within 90 days of hire date.
  • Must have a current CPI certification or receive training within 90 days of hire date.
  • Must have or able to obtain valid Fingerprint Clearance.
  • Must possess reliable transportation, valid driver’s license, and proof of auto insurance.


  • Knowledge of childhood mental disorders, de-escalation techniques, crisis intervention and physical intervention techniques
  • Experience working with poverty level families and drug awareness
